Student Enrollment for New Families

Before new families can register for school they must prove they reside within the District 156 boundaries according to Illinois School Code, State Law, and Board Policy (7:60 Residence).   To assist the District in determining residency and legal custody, the following Student Enrollment Form must be completed, signed, and returned to the District 156 Central Office @ 4716 W. Crystal Lake Road, McHenry, IL 60050 along with the corresponding documents requested.  Students will not be permitted to register for school or select courses until the residency status has been determined.
